out the request form below to have the application form sent out to you.
Notes
- Please read the application form and
instructions carefully.
- Fill out the forms and email or fax it
to the Board's executive officer.
- Remember to send a cheque for $200 plus
GST to cover the application fee or the application will not be considered
by the Board.
